Contact Caris Client Services at 800.901.5177 to arrange for Target Now Complete/Target Now Colon  testing

1. Complete the Target Now Complete/Target Now Colon Tumor Profiling Requisition.
2. Include pathology contact information on the Target Now Complete/Target Now Colon Tumor Profiling Requisition, if known. The Caris Client Services Team will contact pathology as outlined below.
3. Fax Caris Client Services at 866.479.4925 to submit the following:

* Target Now Complete/Target Now Colon Tumor Profiling Requisition
* Insurance information
* Patient clinical note and prior treatment
* Specimen log

Caris Client Services will handle ALL necessary block, slide or fresh tissue coordination with pathology including:
* Shipment of paraffin block/slide or fresh frozen kits including pre-paid shipping labels and delivery of dry ice as needed
* Specimen requirements
* Follow up/tracking on specimen receipt
* Any further authorizations or necessary pathology information
