Contact Caris Client Services at 800.901.5177 to arrange for Caris Target Now testing

 

1. Complete the Caris Target Now Tumor Profiling Requisition.
2. Include pathology contact information on the Caris Target Now Tumor Profiling Requisition, if known. The Caris Client Services Team will contact pathology as outlined below.
3. Fax Caris Client Services at 866.479.4925 to submit the following:

* Caris Target Now Tumor Profiling Requisition
* Insurance information
* Patient clinical note and prior treatment
* Specimen log


Caris Client Services will handle ALL necessary block, slide or fresh tissue coordination with pathology including:
* Shipment of paraffin block/slide or fresh frozen kits including pre-paid shipping labels and delivery of dry ice as needed
* Specimen requirements
* Follow up/tracking on specimen receipt
* Any further authorizations or necessary pathology information
